To best support the broad spectrum of patients we care for, we have a consult-to-treat model. Upon referral to the Union Health Diabetes Clinic, patients will receive a thorough evaluation, care plan, and education. The results of that consultation will be shared with the patient's primary care provider.
We believe this process best supports our patients by creating continuity of care, maintaining clear lines of communication with referring providers and their role as the primary coordinator of care for their patients.
While many patients remain with the practice long-term, some patients transition back to their primary care provider if it is medically appropriate to do so.
