Our Diabetes Clinic in Terre Haute is committed to providing primary care providers the additional support they need for managing their diabetic patients. A strong partnership between area primary care providers and our team will result in the best outcomes for local patients.
In order to best support all our referring physicians and a broad spectrum of diabetes patients, we have a consult-to-treat model. Upon referral to the Union Medical Group Diabetes Team, a patient will receive a thorough evaluation, a care plan, and associated education. The results of that consultation will be shared with the patient's primary care provider.
We believe this process best supports our referring physicians and their role as the primary coordinator of care for their patients.
