What is a swing bed?
Transitional care services (also referred to as swing beds) are a stop between hospital and home, where patients receive the continued care they need while recovering from an illness, injury or surgery. Our skilled staff provides on-site care, 24/7, to help patients return home safely. Patients enjoy the comforts of home, while the family has the comfort of knowing their loved one is receiving the help they need. The length of stay varies, but the quality of care is consistent. After meeting therapy goals, patients can return home stronger and with more confidence.
Why do patients need transitional care services?
Our goal is to help patients return home safely. Transitional care services can help patients who need:
- Rehab or skilled care after a recent hospitalization
- Rehabilitation after surgery
- Therapy after stroke, accident or injury
- Long-term IV antibiotics
- Wound care
Here to help you return home.
When therapy goals are met, we make the transition home easier for patients and their families. As patients leave transitional care services, help may include:
- Providing support if the patient needs home medical equipment and supplies (shower bench, walker, wheelchair, etc.).
- Arranging in-home care if needed.
- Helping find alternative living arrangements.
- Coordinating help through community services and resources.
How do I know if I’m a good fit for transitional care?
Eligibility is based on a specific set of criteria. Contact a case manager today who can work with you to identify the best health solution for you.
At Union Hospital Clinton, we offer therapy in the following services as needed:
- Pulmonology, Nephrology & Cardiology Consultations
- Respiratory
- Physical Therapy
- Speech Therapy
- Wound Therapy
- Occupational Therapy
Transitional care patients benefit from a quality care model that includes the following:

One-to-one nurse to patient ratio
- Highly-skilled nursing staff (RN and above)
- Call light answers in 10-15 seconds
- Private rooms
- 24-hr respiratory therapist
- Flexibility to leave with family and return by EOD
- Therapy available seven days per week
- Transport is not included, but can be arranged
- 100% covered by Medicare
- 100% covered by most insurance
Prospective patients do not qualify for our program if they fall into the following categories:
- Medicaid patient
- Requires more than 10 days of skilled nursing
- Requires one or more of the following:
- Locked unit
- Psychiatry service
- o 24-hr sitter needs
- Requires more than 30 minutes per day of therapy
- Requires specialty medications
