The Anticoagulation Clinic at Union Hospital is run by experienced clinical pharmacists trained in Coumadin (warfarin) management.

  • Comprehensive education and counseling regarding your Coumadin therapy
  • Review of current medications, including over-the-counter medications for potential drug interactions
  • Bridging Coumadin therapy around surgery or other invasive procedures
  • Point of Care INR (blood test used for monitoring) testing using the finger-stick method, providing results in minutes
  • Dose adjustments
  • Blood pressure and heart rate are monitored on each visit.
